Zusammenfassung

Knochentumor mit intramedullärer, gewöhnlich diaphysärer Ausdehnung und transkortikalem Weichteildurchbruch, von gewöhnlich harter Konsistenz mit weicheren hämorrhagischen und zystisch degenerierten Arealen. Histologisch uniformes klein-, blau-, rundzelliges Bild mit mäßiger Mitosenzahl. Vereinzelt unvollständig pseudorosettenartige Anordnung der Tumorzellen. In der PAS-Färbung gewöhnlich Nachweis reichlicher intrazytoplasmatischer Glykogenablagerungen. Immunhistochemisch Positivität mesenchymaler Marker, z. B. Vimentin; variabler Nachweis neuronaler Differenzierung: NSE, Leu7, PGP9.5, S100; daher unscharfe Abgrenzung gegenüber malignen peripheren neuroektodermalen Tumoren (MPNT, PNET). Diese beiden Tumoren werden heute als unterschiedliche Differenzierungen desselben Malignoms angesehen. Die Definitionen sind noch nicht vereinheitlicht. Vorgeschlagen ist, als Ewingsarkom den klein-, rundzelligen Knochentumor zu definieren, der bis zu einem neuronalen Marker exprimiert und als PNET alle klein-, rundzelligen Tumoren mit mehr als einem neuronalen Marker. Die Proliferationsaktivität, immunhistochemisch nachweisbar mit proliferationsassoziierten Kernantigenen, z. B. Ki-67, ist relativ gering und liegt zwischen 10 % und 15 %.
