Zusammenfassung

Unter einer Herzinsuffizienz versteht man eine eingeschränkte körperliche Belastbarkeit aufgrund einer nachweisbaren kardialen Funktionsstörung (WHO-Definition). Bei Patienten mit Herzinsuffizienz besteht ein Mißverhältnis zwischen der geförderten Blutmenge und dem der jeweiligen Situation entsprechenden Blutbedarf. Bei einem Krankheitsverlauf über Wochen und Monate handelt es sich um eine chronische Herzinsuffizienz, im Gegensatz zur akuten Herzinsuffizienz, bei der die kardialen Funktionsstörungen mit entsprechender Symptomatik im Verlauf von Minuten bis Stunden auftreten. Die chronische Herzinsuffizienz ist die häufigste kardiale Erkrankung. Wegen der schlechten Prognose im Stadium der manifesten Herzinsuffizienz NYHA III und IV (Letalität 20–40 %/Jahr) ist eine früühzeitige Diagnostik und Therapie notwendig (Übersicht 1).
