Zusammenfassung

Unbehandelt verläuft die mikrobiell verursachte Endokarditis (IE) letal.Verzögerungen in der Diagnostik und Therapiefehler werden regiehaft mit einer erheblichen Prognoseverschlechterung bezahlt. Eine IE ist bei allen Patienten mit unklarem Fieber oder nachgewiesener Bakteriämie — insbesondere von Streptokokken, Enterokokken und Staphylokokken — oder bei Neuauftreten eines Herzgeräuschs zu vermuten und unverzüglich mittels Echokardiographie abzuklären. Bei vermuteter oder gesicherter IE ist eine enge Kooperation zwischen Kardiologie, Mikrobiologie und Kardiochirurgie unabdingbar. Für die Mehrzahl der heute Erkrankten wird eine chirurgische Intervention noch während der akuten Infektion erforderlich.
