Zusammenfassung
Myelodysplastische Syndrome (MDS) bilden eine heterogene Gruppe erworbener klonaler Stammzellerkrankungen, die bevorzugt im höheren Lebensalter auftreten (Altersgipfel 60–70 Jahre). Leitbefund der Krankheitsgruppe ist eine periphere Mono-, Bi- oder Panzytopenie bei normalem oder erhöhtem Zellgehalt des Knochenmarks. Die Verminderung peripherer Blutzellen resultiert aus einer schon morphologisch erkennbaren Reifungsstörung der Hämatopoese, die im klassischen Fall alle 3 blutbildenden Zellsysteme betrifft.
MDS sind relativ häufige Erkrankungen. Die jährliche Neuerkrankungsrate in der Bevölkerung wird auf 4–13/100.000 geschätzt. Jenseits des 70. Lebensjahrs betragen die Inzidenzraten 20–50/100.000 pro Jahr (Aul et al. 1992). Aufgrund der zunehmenden Überalterung der Bevölkerung muss in den nächsten Jahren mit einem weiteren Krankheitsanstieg gerechnet werden. Haupttodesursachen sind infektiöse und hämorrhagische Komplikationen sowie Transformation in akute myeloische Leukämien (AML). Derzeit sind bei der International MDS Foundation 295 Studien registriert, die sich v.a. mit einer Verbesserung der Behandlungsmöglichkeiten myelodysplastischer Syndrome beschäftigen.
