Zusammenfassung

Der Darm ist das Organ des Menschen, das als „innere Haut“ unmittelbar mit der Umwelt kommuniziert.Auf eine Vielzahl endogen oder exogen verursachter Störungen reagiert der Darm mit einem beschränkten Repertoire klinischer Symptome. Subjektiv ist der Darm dabei oft eine Schaltstelle zwischen Körper und Seele. Die klassischen Darmsymptome wie z. B. Durchfall, Verstopfung, Bauchschmerz oder Völlegefühl werden dem Patienten sofort leidvoll bewusst. Es darf -davon ausgegangen werden, dass jeder Mensch in seinem Leben die Folgen einer Störung des Darms irgendwann zu spüren bekommt. In sog. Entwicklungsländern ist z. B.die Diarrhö eine häufigeTodesursache; weltweit, so auch in westlichen Industrienationen, ein wichtiger Grund für Arbeitsausfall. Ein Viertel der deutschen Bevölkerung leidet an einer chronischen Erkrankung des Darms.

Vor allem die Ursachen der entzündlichen, tumorbedingten und durch Motilitätsstörung charakterisierten Darmerkrankungen sind trotz intensivster Forschungsbemühungen und -erfolge leider noch unzureichend geklärt, sodass dieTherapie vielfach empirisch erfolgt.

Auf dem Gebiet der chronisch entzündlichen Darmerkrankungen wurden in den letzten Jahren Suszetibilitätsloci für den Morbus Crohn und die Colitis ulcerosa ent-deckt. 2001 konnten Polymorphismen des NOD2-Gens als Suszeptibilitätsmarker für den Morbus Crohn (bei einem Teil der Patienten) identifiziert werden. Möglicherweise wird die Assoziation eines bestimmten Genotyps (z. B.NOD2-Mutation) und distinkter klinischer Phänotypen (z.B. Ileumbefall, stenosierender Verlauf) bei chronisch entzündlichen Darmerkrankungen eine Diversifizierung der Krankheitsentitäten herbeiführen und zukünftig eine gezielte therapeutische Intervention auf genetischer Basis ermöglichen. Ärztliche Unkenntnis und praktische Ratlosigkeit herrschen vielfach beim Reizdarmsyndrom bzw.auf dem Gebiet der Motilitätsstörungen, obwohl auch hier durch Entwicklung von 5-Hydröxytryptamin4-Rezeptor-Agonisten und 5-Hydroxytryptamin3-Rezeptor-Antagonisten Fortschritte gemacht wurden.Ver-bessert wurde auch dieTherapie kolorektaler Karzinome durch Einsatz neuer Chemotherapeutika. Zu begrüßen ist, dass auch die Maßnahmen zur Prävention kolorektaler Adenome und Karzinome intensiviert wurden durch Erstellung Patientengruppen-spezifischer Leitlinien, medienwirksame breite Aufklärung (z. B.durch die Gastro-Liga e.V.) und schließlich die im Frühjahr 2002 in das kassenärztliche Programm auf genommene Vorsorgekoloskopie für Normalpersonen ab dem 55. Lebensjahr.
