Zusammenfassung
Es gibt eine Vielzahl von Erkrankungen des retikulohistiozytären Systems mit einer sehr breit gefächerten Röntgensymptomatik, daher ist es auch außerordentlich schwer, einzelne dieser Erkrankungen in eine röntgenologische Systematik (z. B. Erkrankungen mit Dichtezu- oder -abnahme, mit osteolytischen Veränderungen etc.) zu bringen. Sieht man einmal von der Mastozytose ab, so ist das wesentliche ätiologische Moment der Erkrankungsgruppe in einer mehr oder weniger bekannten und gesicherten Stoffwechselstörung zu suchen, die offensichtlich zu einer reaktiven Makrophagenwucherung (Histiozyten, Monozyten) als Antwort auf eine Speicherung intermediärer Stoffwechselprodukte führt. Die Makrophagenwucherung kann grundsätzlich in allen Organen oder Organsystemen des RES auftreten. Bei den einzelnen hier zur Diskussion stehenden Erkrankungen konzentriert sie sich jedoch vor allem auf Milz und Leber, Lymphknoten und Haut sowie insbesondere auf das Knochenmark. Das proliferierende retikulohistiozytäre Gewebe verdrängt im Knochenmark das normale blutbildende Markgewebe und führt zu einem gestörten Knochenumbau mit fokalem oder diffusem Ab- und reaktivem Anbau, der je nach Grundleiden enzymatisch und/oder hormonell beeinflußt wird (z. B. durch Heparin oder Prostaglandine). Aus dem Gesagten leitet sich die oben angesprochene bunte Röntgensymptomatik ab, die aus einer mehr oder weniger deutlichen Osteoporose, aus Osteolysen und einer Osteosklerose besteht.
