Zusammenfassung
Die Schockforschung der letzten 50 Jahre hat zunehmend deutlich gemacht, daß initial nicht Störungen der Makrozirkulation, sondern vielmehr die der Mikrozirkulation für die nutritive Mangelversorgung von Organen, die zelluläre Hypoxie und den Funktionsverlust von Zell- und Organsystemen im Schock sowie schließlich den Tod veantwortlich sind. Einigkeit besteht heute auch darin, daß die lokale Regulation der Mikrozirkulation stark abhängig von der Freisetzung bzw. Bildung chemischer Mediatoren ist, die entweder aus dem Gewebeparenchym in unmittelbarer Nähe der kleinen Blutgefäße, direkt aus dem Komplex glatte Muskelzellendothelzelle oder den Blutzellen selbst stammen [12–14]. Bei der Beschreibung von Störungen der Mikrozirkulation und damit auch von Ursache-Wirkungs-Prinzipien in der Pathogenese besonders des septischen Schocks, gewinnen deshalb in den letzten 15 Jahren verschiedene Mediatorkonzepte zunehmend an Bedeutung. Die Einschätzung der Rolle der verschiedenen Mediatoren und anderer aktiver Substanzen variiert in vielfältiger Weise und ist v. a. abhängig vom Schockparadigma des Wissenschaftlers, dem Stadium der Schockprogression (d. h. der Organinsuffizienz) sowie der untersuchten Spezies. Im folgenden soll der Versuch einer Standortbestimmung unternommen und Strategien zur Sicherung eines einzelnen Mediators aufgezeigt werden. Abschließend wird ein neues Kausalitätsmodell zur Einschätzung der relativen Bedeutung eines einzelnen Mediators in der Pathogenese des septischen Schocks beschrieben, von dessen Anwendung verbesserte therapeutische Konzepte abgeleitet werden können.
