Zusammenfassung
Das derzeit richtungsweisende Klassifikationssystem der Internationalen Headache Society (IHS 1988; Göbel et al. 1991) beschreibt als Gruppe 2 „tension type headache“, also Kopfschmerz vom Spannungstyp (KST). Wie auch die anderen 13 definierten Kopfschmerzformen wird KST für den klinischen und wissenschaftlichen Gebrauch phänomenologisch exakt operationalisiert. Unterschieden wird zwischen episodischem und chronischem KST. Episodischer KST besteht in wiederkehrenden, Minuten bis Tage dauernden Kopfschmerzepisoden. Die Schmerzqualität ist drückend bzw. spannend, die -intensität schwach bis mittel. Der Schmerz wird vom Patienten bilateral lokalisiert und verschlimmert sich bei körperlichen Alltagsaktivitäten nicht. Licht-oder Lärmempfindlichkeit können vorkommen, nicht aber Übelkeit. Wer an episodischem KST leidet, erlebt im Jahr mehr Tage ohne als mit Schmerz. Bei Patienten, die an chronischem KST leiden, überwiegen dagegen die Tage mit Kopfschmerz. Außerdem kann Übelkeit als Symptom auftreten, nicht aber Erbrechen. Ansonsten gilt für den chronischen die Beschreibung des episodischem KST. Episodischer und auch chronischer KST können mit oder ohne eine Störung der perikranialen Muskulatur auftreten. Eine solche Störung liegt vor, wenn die Empfindlichkeit der perikranialen Muskulatur — nachzuweisen durch Palpation oder Druckalgometer — oder wenn das EMG dieser Muskulatur in Ruhe oder während physiologischer Tests erhöht ist.
