Zusammenfassung
Mit einer Inzidenz von 2–5% aller Magentumoren ist das primäre Magenlymphom ein seltener Tumor. Neben den extranodalen Non-Hodgkin-Lymphomen des Magens, die in hoch-und niedrigmaligne Formen eingeteilt werden, findet sich in 40% der Fälle das Magenlymphom vom MALT-Typ (mucosa associated lymphoid tissue). Diese MALT-Lymphome entstehen fast immer auf dem Boden einer chronischen Infektion mit Helicobacter pylori. Ein Therapieversuch durch Eradukation des H. pylori ist nur bei niedrigmalignen MALTLymphomen im frühen Stadium (E I1) mit enger Führung der Patienten zu empfehlen. Die Therapie der Magenlymphome hängt einerseits von der Ausbreitung der Krankheit, andererseits entscheidend von der histopathologischen Klassifikation ab. Hochmaligne Lymphome müssen — auch nach kompletter Resektion — generell chemotherapeutisch behandelt werden. Bei der Therapie der Magenlymphome ist eine enge Kooperation zwischen Chirurgen, internistischen Onkologen und Strahlentherapeuten erforderlich.
