Zusammenfassung
Die Lunge ist der häufigste Sitz von Metastasen. Diese finden sich meist in Verbindung mit einem fortgeschrittenen Tumorleiden. Ist die Metastasierung auf die Lunge beschränkt, so haben chirurgische Verfahren ihre Berechtigung im onkologischen Konzept. Resektionen in kurativer Absicht setzen jedoch einen günstigen Lokalbefund (limitierte Anzahl kleinerer Herde im Lungenmantel) und extrathorakale Tumorfreiheit voraus. Der Standardeingriff ist die parenchymsparende atypische Resektion. Wichtigste prognostische Faktoren sind die Herkunft des Primärtumors und die Vollständigkeit der Resektion. Chemotherapiesensible Metastasen werden nur nach unvollständiger Remission der Operation zugeführt. Selten erfolgen Operationen in diagnostischer oder palliativer Absicht.
