Zusammenfassung
Unerwünschte Arzneimittelwirkungen treten häufig auf. Es wird geschätzt, dass etwa 10–20% der hospitalisierten Patienten davon betroffen sind. Die häufigste Ursache für unerwünschte Arzneimittelwirkungen ist eine mangelhafte Dosisindividualisierung. 80% aller unerwünschten Arzneimittelwirkungen gehen auf eine fehlerhafte Dosierung zurück; sie sind theoretisch alle vermeidbar. Eine häufige Ursache für Dosierungsfehler besteht in der Verabreichung normaler Dosierungen an Patienten mit normalem Plasmakreatinin. Ein normales Plasmakreatinin schließt wegen des „Kreatininblinden“ Bereichs (GFR 100–50 ml/min) eine Nierenfunktionseinschränkung nicht aus. Insbesondere bei älteren Patienten, die eine geringe Muskelmasse haben und folglich eine geringere Menge an Kreatinin produzieren, ist ein sog. „normales” Plasmakreatinin bereits mit einer Einschränkung der Nierenfunktion verknüpft.
