Zusammenfassung

Adrenalin ist ein im Nebennierenmark gebildetes Katecholamin mit sowohl α-(vasokonstriktorischer) als auch β1-(positiv inotroper und positiv chronotroper) und β2-mimetischer (vaso- und bronchodilatatorischer) Wirkung. Es stimuliert alle peripheren sympathischen Rezeptoren, hat jedoch keinen zentralen Angriffspunkt, da es nicht die Blut-Hirn-Schranke passiert. Therapeutischen Einsatz findet Adrenalin im Rahmen der kardiopulmonalen Reanimation, insbesondere bei Herzstillstand infolge von Kammerflimmern oder bei pulsloser ventrikulärer Tachykardie, Asystolie, elektromechanischer Entkopplung, bei profunder Bradykardie sowie des Weiteren in der Akuttherapie des anaphylaktischen Schocks und als Konstriktorzusatz für Lokalanästhetika [1]. In der Intensiv- und Akutmedizin bedeutsam sind die folgenden kardiozirkulartorischen Effekte von Adrenalin:

  • Anstieg des peripheren Widerstandes

  • Anstieg des systolischen und diastolischen Blutdrucks

  • Anstieg der elektrischen Aktivität und Automatie des Myokards

  • Anstieg der koronaren und zerebralen Durchblutung

  • Anstieg der myokardialen Kontraktilität und Erregungsleitung

  • Anstieg des myokardialen Sauerstoffbedarfs
