Zusammenfassung

Für diese sog. funktionellen gastrointestinalen Beschwerden, die durch komplexe Interaktionen zwischen Ga-strointestinaltrakt und ZNS verursacht werden, gibt es keinen pathophysiologischen Marker und daher keinen diagnostischen Test. Gastroenterologen finden keine pathologischen Organveränderungen. Für Erwachsene mit Colon irritabile wurden zur besseren Klassifikation Kriterien entwickelt, die zumindest für Schulkinder und Jugendliche auch Verwendung finden könnten: über mindestens 3 Monate rekurrierende Bauchschmerzen, die sich auf Defäkation bessern und mit Stuhlunregelmäßigkeiten hinsichtlich Frequenz und Konsistenz verbunden sind. Öfter als bei organisch faßbaren Krankheiten finden sich bei diesen Menschen Tenesmen, Schleimabgang, das Gefühl inkompletter Entleerung, Blähungen und Flatulenz, gelegentlich auch Sodbrennen, Dysphagie und biliäre Dyskinesie. Die rezidivierenden Bauchschmerzen treten typischerweise nur tagsüber und periumbilikal auf, manchmal steht eine Obstipation im Vordergrund.
