Zusammenfassung
In der Skalpellchirurgie ist es für die spätere Kosmetik einer Operationsnarbe von besonderer Wichtigkeit, daß der Hautschnitt möglichst senkrecht zur Hautoberfläche erfolgt (Abb. 13). Dieses Vorgehen wird durch eine gleichmäßige artefizielle Spannung des Operationsgebietes während des Schnittvorgangs erleichtert. Nach der Einzeichnung des Operationsplans beginnt der Hautschnitt an jener Stelle, die die größte Präzision erfordert (z. B. Gehörgang, Tränenpünktchen etc.) und wird nach Möglichkeit in einem Zug vollendet. Dadurch wird ein Verwischen der präoperativen Farbstoffmarkierung durch die operationsbedingte Blutung weitgehend vermieden. Die umschnittene Läsion kann anschließend an einer Stelle mittels eines Hauthäkchens oder einer feinen Pinzette angehoben und die tieferliegenden Strukturen mit dem Skalpell oder der Präparierschere durchtrennt werden. Eine exakte Blutstillung ist dabei unerläßlich. Blutende Gefäße werden einzeln abgeklemmt und mit atraumatischem resorbierbaren Nahtmaterial (Stärke 3-0 bis 5-0) umstochen, ligiert oder mit dem Elektrokauter verschorft.
