Zusammenfassung
In der postnatalen Phase ist der Kreislauf erheblichen Umstellungsvorgängen unterworfen. Dadurch werden z.B. einige in utero gut tolerierte angeborene Herzfehler unmittelbar post partum symptomatisch. Bedeutsam ist hierbei außerdem, daß das Herz des Neugeborenen schon physiologischerweise auf der Ebene eines hohen enddiastolischen Füllungsvolumens arbeitet [45]. Ferner ist die myokardiale Katecholaminkonzentration in den ersten Lebenstagen besonders niedrig [26]. Eine zusätzliche Belastung, die später ohne weiteres kompensiert wird, kann daher beim Neugeborenen rasch zur Herzinsuffizienz führen.
