Zusammenfassung

Trotz der im vorstehenden Kapitel geschilderten positiven Aspekte stellt die Einführung der Intensivmedizin in die Neonatologie Ärzte und Pflegepersonal oft vor schwere ethische Entscheidungen [12, 19, 23, 24, 32]: Sind bei jedem Früh- und Neugeborenen alle Möglichkeiten einer intensivmedizinischen Behandlung auszuschöpfen? Gibt es eine „biologische Grenze” für den sinnvollen Einsatz einer künstlichen Beatmung bei sehr unreifen Frühgeborenen? Ist eine künstliche Beatmung bei einem Frühgeborenen mit tamponierender Ventrikelblutung (Grad 4) ohne Anzeichen von Spontanatmung fortzusetzen? Wie lange soll bei schwerer bronchopulmonaler Dysplasie und progredientem Cor pulmonale die Beatmung weitergeführt werden? Soll bei einem Neugeborenen mit Down-Syndrom und Pulmonalatresie Prostaglandin E1 infundiert werden? Verpflichtet uns unser ärztliches Ethos nicht in bestimmten Fällen, etwa bei einem Neugeborenen mit gesicherter schwerer zerebraler Fehlbildung, zum Abbruch einer intensivmedizinischen Behandlung? Wann ist es geboten, eine von den Eltern abgelehnte Behandlung durch vormundschaftsgerichtlichen Beschluß zu erzwingen?
