Zusammenfassung

Frühgeborene geraten in den ersten beiden Lebenswochen häufig in eine negative Natriumbilanz, da sie über die Niere viel NaCl verlieren und über den Magen-Darm-Kanal wenig resorbieren [2, 3]. Trotz hoher Aldosteronsekretion [21] und maximaler Stimulation ist die Natriumrückresorption im proximalen und distalen Tubulus ungenügend. Der tägliche Natriumbedarf beträgt 3–5 mmol/kg/Tag [4, 8]. Andererseits geraten diese Kinder durch erhöhten transepidermalen Flüssigkeitsverlust leicht in eine hyperchlorämische Azidose, wenn ihnen viel Natriumchlorid zugeführt wird, insbesondere wenn sie nicht in angefeuchteten Inkubatoren gepflegt werden, oder wenn sie Phototherapie erhalten (s. 16.9).
