Zusammenfassung
Die operative Primärtherapie des Mammakarzinoms hat in den vergangenen 30 Jahren wesentliche Entwicklungen durchgemacht. Grundlage dafür war die sich wandelnde Einschätzung der Biologie dieser Karzinomerkrankung. Die von Rotter und Halsted Ende des letzten Jahrhunderts empfohlene Radikaloperation des Mammakarzinoms (radikale Entfernung der Brustdrüse unter Mitnahme großer Hautareale und komplette Resektion des großen und kleinen Brustmuskels sowie der axillären Lymphknoten) ging von der Vorstellung aus, dass das Mammakarzinom ein lokal fortschreitender Tumor sei, der möglichst radikal im Gesunden exstirpiert werden müsse (Halsted 1894). Anfang der 7oiger Jahre dann wurden insbesondere von Veronesi aus Mailand Daten publiziert, die zeigten, dass auch ultraradikalere Operationsformen, die über das Konzept von Rotter und Halsted hinausgingen, keine Verbesserung der Überlebenszeiten für die Patientinnen erbrachten (Lacour et al. 1976).
