Zusammenfassung
Kein anderes Konzept hat in den letzten 20 Jahren in der Laienpresse, aber auch in Medizin und Psychologie eine so breite Beachtung gefunden wie die Streßhypothese und — in enger Verwandtschaft hierzu — die Hypothese zum Typ-A-Verhalten. Hunderte von Autoren haben sich in nicht mehr überschaubaren Publikationen mit diesen Hypothesen befaßt. Für die meisten Autoren scheinen, gestützt auf die Rut der scheinbar positiven Befunde, Streß und Typ A zu unumstößlichen Risikofaktoren für viele Erkrankungen, speziell jedoch für die koronare Herzkrankheit, geworden zu sein. Bei den Laien ist diese Überzeugung noch ausgeprägter. Bei einer repräsentativen Umfrage der Bevölkerung im Jahre 1982 stimmten 52% der Befragten im Alter von 16–80 Jahren der Frage “Ich habe häufig das Gefühl, im Streß zu sein” zu (Fahrenberg et al. 1984). Bei einer Stichprobe von 87 Herzinfarktpatienten, die nach den vermuteten Ursachen ihrer Erkrankung gefragt wurden, erreichten die Ursachen Zeitdruck und berufliche Belastung die höchsten Skalenwerte, die somatischen Risikofaktoren nahmen dagegen nur untere bis mittlere Ränge ein (Fahrenberg et al. 1985). In einer späteren Untersuchung an 124 Koro-narkranken konnte dieses Ergebnis bestätigt werden (Kreutel 1989). In diesem Beitrag soll dargelegt werden, inwieweit die verbreitete Überzeugung, Streß und Typ-A-Verhalten seien wichtige Risikofaktoren der koronaren Herzkrankheit, durch prospektive Untersuchungen gestützt wird.
