Zusammenfassung

Die intraartikuläre Substitution des vorderen Kreuzbandes aus einem Teil des Lig. patellae ist seit Bruckner (1966) und Jones (1963, 1970) eine etablierte Rekonstruktionstechnik. Von Palmer 1938 erstmals empfohlen, wurden seither durch zahlreiche klinische und experimentelle Studien (Alm u. Strömberg 1974; Arnoczky et al. 1982; Butler et al. 1979; Clancy et al. 1981; Drobny et al. 1984; Jakob 1985; Kieffer et al. 1984; Noyes et al. 1984; Odensten u. Gillquist 1985; Paulos et al. 1983; Scapinelli 1968; Shino et al. 1984) die histomorphologischen und biomechanischen Grundlagen der Methode erarbeitet. Basierend auf den Beobachtungen von Scapinelli (1968) zum Gefäßmuster des menschlichen Kniegelenkes untersuchten Alm u. Strömberg (1974), Arnoczky (1983) und Arnoczky et al. (1982), Clancy (1983) und Clancy et al. (1981) sowie Drobny et al. (1984) die Revaskularisation des Lig.-patellae-Transplantats nach primär erhaltener und nach unterbrochener Durchblutung. Einen wesentlichen Beitrag erbrachten Butler et al. (1979), denen der Nachweis gelang, daß das Lig. patellae die mechanisch stärkste Struktur der sich zur Substitution anbietenden biologischen Gewebe darstellt. Zudem wird durch die Entnahme eines Streifens aus dem zentralen Drittel des Lig. patellae keine für die Kniestabilität essentielle Struktur geschwächt.
