Zusammenfassung
Die Sarkoidose ist eine granulomatöse Systemerkrankung unbekannter Ätiologie. Die Lungen mit hilären und mediastinalen Lymphknoten sind bevorzugt befallen, häufig finden sich auch Augen- und Hautveränderungen. Die Diagnose gilt als gesichert, wenn typische klinisch-radiologische Befunde durch den Nachweis nichtverkäsender epitheloidzelliger Granulome bestätigt werden. Charakteristische immunologische Befunde sind die Unterdrückung der Tuberkulin- und anderer Hautreaktionen vom verzögerten Typ, verbunden mit einer verstärkten zellulären Immunreaktion vom TH-1-Typ in den befallenen Organen. Ein akuter Krankheitsbeginn mit Erythema nodosum oder die Erstmanifestation als asymptomatische bihiläre Lymphadenopathie ist meist mit einer selbstlimitierenden Verlaufsform verbunden, während ein schleichender Beginn, besonders in Verbindung mit multiplen extrapulmonalen Organmanifestationen, zur progressiven Fibrose der Lunge und anderer Organe führen kann. Die erste Erwähnung einer Hautsarkoidose geht auf J. Hutchinson 1877 zurück. 1899 beschrieb C. Boeck Hautveränderungen als „multiples benignes Sarcoid“ (Sarcoid → Sarkoidose).
