Abstract
Mit seinem Thema Hypospadie legt der Autor ein außergewöhnliches Maß an Fachkenntnis, an Engagement und persönlicher Erfahrung in seinen Beitrag hinein.
Wie die Hemmungsfehlbildung, was Hypospadie ist, embryonal zustande kommt, wissen wir nicht. Diskrete Erblichkeit besteht. Hypospadie erscheint in vielen Variationen. Sie zu klassifizieren geschieht am besten nach dem Mündungsort der Urethra, jedoch darin verbindlich erst nach der sog. Aufrichtung, weil die Mündung damit immer weiter nach proximal gerät. Mithin heißt die Klassifizierung: periglandulär — Schaftmitte — penoscrotal. Peripher des oft verengten Orifizium externum fehlt die Urethra samt Corpus spongiosum. Was so fehlt, imponiert als sog. Chorda. Die zugehörigen Corpora cavernosa sind unterentwickelt, die Penishaut darüber verdünnt.
Penoscrotale Hypospadie zusammen mit nicht palpablen Testes zwingt zur Abklärung einer Zwitterformation mittels Chromosomenanalyse, dies sogar auch bei palpablem Skro-talinhalt. Falls nicht sicher maskulin, empfiehlt sich die Umwandlung nach feminin.
Mit Hypospadie assoziiert gibt es Anomalien der distalen und proximalen Harnwege, betont Kryptorchismus, auch vesikorenale Refluxivität,
Die Indikation zu plastischer Korrektur ist bei jedem peripheren oder proximalen Orifizium gegeben. Bei Hypospadia glandis und coronaria ist sie davon abhängig zu machen, ob eine Peniskrümmung besteht, worüber verbindlich nur artifizielle Erektion mittels Kochsalzinjektion in ein Korpus informiert.
Das bestgeeignete Lebensalter ist Ende vom ersten auf das zweite Lebensjahr, während des sog. „psychischen Fensters“. Einbindung der Eltern ist immer notwendig.
Das Operationsziel ist außer Zweifel der künftige Anspruch einer geordneten Kohabitation und Verlegung des Orifizium externum an die Spitze der Glans penis.
Die Operationsmethodik kennt zahlreiche Verfahren. Viele berühmte Namen sind darin verbunden. Einzeitige Aktion anstelle mehrzeitiger gilt heute als bisher höchste Entwicklung. Dem Operateur werden überdurchschnittliche Kenntnisse und Fähigkeiten abverlangt. Daher muß es auch Selbstbeschränkungen in der Kollegenschaft geben. Höchst atraumatisches Operieren, zugleich feinstes Instrumentarium und Nahtmaterial einschließlich Lupenvergrößerung gehören dazu. Jede Hypospadieplastik beginnt mit der Umschneidung des Meatus und Rückverlagerung bis zu kompletter Streckung des Penisschaftes. Überprüfung mittels artefizieller Erektion ist unverzichtbar. Nicht immer gibt es exzidier-bares Substrat. Alle Methodik geht darauf hin, eine funktionierende Urethra neu zu bilden, alles sind Variationen der Plastik von Denis Brown und seiner Vorgänger. Zwischenschaltung von Anteilen aus Blasen- und Mundschleimhaut hat Vor- wie Nachteile. Die Hypospadia sine Hypospadie kann nur das bekannte Nesbit-Verfahren beheben. Der Autor empfiehlt temporär transurethrale Katheterableitung anstelle suprapubischer. Fortbestehende erektive Penisschaftkrümmung und Striktur der neuen Harnröhre sind fallweise schwerwiegende Komplikationen, und sie belasten die Operationstechnik (Schwenklappenplastik und asymmetrischer Nahtverschluß nicht erwähnt). Stichworte: Sequenz von Detaillösungen, komponentenorientiertes Vorgehen.
Seine Ergebnisse: Therapieziel voll erreicht in 95 % bei distaler Hypospadie, in 93 % bei medialer und in 76 % bei proximaler. n=400 (1995). Inzwischen n=i350 (1999). Wissenschaftlich und klinisch eine souveräne Einzelleistung.
