Abstract
Dieses Thema der Urologie, Kinder wie Erwachsene, gehört, was die Indikation betrifft, zu den schwierigsten. Die verfügbaren Methoden sind für beide Altersgruppen die gleichen. Aus benigner Indikation überwiegen die Kinder, aus maligner die Erwachsenen.
Harnableitung oder auch Harnumleitung hat zwei Aufgaben: die Desobstruktion obstruierter Harnwege und den Ersatz für eine nicht mehr verfügbare Harnblase. Desobstruktion dann, wenn ein einfacher Splint nicht genügt, dazu dann die Entscheidung, ob temporär, das häufigere, oder permanent, das schwerwiegendere. Schwieriger und aufwendiger wird das Thema, wenn der Rückweg in die Harnblase nicht mehr begehbar ist, ein Entschluß, dem psychologisch Resignation wie Widerstand eigen ist. Harnumleitung in die Bauchdecke, möglich mittels eines Conduits aus Dünn- oder Dickdarm, beides zu ihrer Zeit großartige Fortschritte, die jedoch inzwischen von Besserem abgelöst sind, ähnlich auch darin der noch ältere Ausweg der Harnableitung in den Dickarm zur weiteren Darmpassage.
Die Ersatzblase aus Intestina mit Ventil zur Bauchdecke, einem großteils kontinenten, war ein großer Fortschritt, noch mehr der Anschluß solch einer Darmersatzblase an die natürliche Harnröhre, bei Männern einfacher als bei Frauen. Ein deutlich kleineres Problem daneben ist die Vergrößerung einer zu klein gewordenen Harnblase mittels eines Intestinalabschnittes. Der Indikationsbogen geht also von Desobstruktion der Harnwege über Substitution zu Augmentation der Harnblase.
Kinder sind in der Indikation fast immer nur Subjekt, ihr Objekt sind die Ärzte und die Eltern. Die Akzeptanz macht wenig Denkarbeit im Falle der temporären Ableitungsmethoden. Sie sind lange bekannt und viel seltener indiziert, seit es die perkutane Nephrostomie gibt. Auch die bei Kindern früher häufigere Indikation zu definitiver Harnableitung mittels eines Conduits ist fast gänzlich aufgegeben, weil diese Methodik, nicht nur nasses Stoma, den weiteren Niedergang der Niere nur verzögern, aber nicht verhindern konnte. Deshalb ist der Blasenersatz auch bei Kindern ein wichtiges Thema geworden. Die Varianten dazu, Ileozökalpouch oder Ileoneoblase, stehen sich an Wertigkeit nicht viel nach. Weitgehende Kontinenz des Stomas bedeutet großen Fortschritt, noch mehr der Anschluß der neuen enteralen Blase an die natürliche Harnröhre. Beides verlangt viel Operationskunst. Komplikationen und Revisionen sind keine Seltenheit. Männliche Anatomie bringt mehr Vorteile als weibliche.
Pathophysiologisch gesehen sind die Conduitmethoden optimal, weil Niederdrucksystem, sofern es so bleibt. Die permanente Nässe trotz vorteilhafter Auffangeinrichtungen bleibt der große Nachteil. Umgekehrt liegt der große Vorteil der kontinenten Ersatzblase auf der Hand. Ihre Nachteile jedoch sind vielfältig. Zunächst deren lange fortgesetzte natürliche Dünndarmsekrete, weiter das Problem der Resorption, das jedoch insgesamt weniger gravierend ist, als primär zu erwarten war. Schließlich vor allem bei Verwendung von Dickdarmanteilen die Chance der Tumorinduktion nach Jahr und Tag.
Das Kapitel spiegelt alles, was es an Höhen und Tiefen in der Erwachsenen- wie Kinderurologie gibt. Die Lernphase von der Erwachsenen- zur Kinderurologie birgt weniger Klippen als der direkte Zugang in die Kindermorbidität. Einprägsame Tabellen und Abbildungen vervollständigen die Abhandlung. Aus jedem Satz geht die enorme Erfahrung der Ulmer Klinik hervor, in der die Autoren tätig sind. Pathophysiologische Grundlagenkenntnis und operativer Hochstand gehen eine optimale Verbindung ein.
