Zusammenfassung
Die Anzahl der diagnostischen Eingriffe nimmt dank der modernen Methoden, die heute zur Verfügung stehen, ständig zu. Viele dieser Eingriffe können — v. a. bei jüngeren Kindern — nur unter Sedation oder Anästhesie durchgeführt werden. Da Anästhesisten häufig für die Betreuung dieser Kinder herangezogen werden, müssen sie mit den Schwierigkeiten, die auftreten können, vertraut sein. Die rein anästhesiologischen Probleme stehen dabei weniger im Vordergrund als die organisatorischen und örtlichen Gegebenheiten. Fachlich steht die Tatsache im Zentrum, daß es sich bei der Bewußtseinsveränderung zwischen einem wachen Kind und einem anästhesierten Kind um ein Kontinuum handelt. Leichte Sedation, tiefe Sedation, leichte Anästhesie, oberflächliche Anästhesie, tiefe Anästhesie usw. sind relativ unspezifische Beschreibungen dieses Kontinuums. Zentral bei der Beurteilung des Sedationszustands ist, ob das Kind in der Lage ist, die Atemwege offenzuhalten und adäquate Abwehrreflexe aufrechtzuerhalten. Diese Beurteilung ist im Einzelfall allerdings schwierig und kann sich jederzeit ändern, so daß die unten aufgeführten Begriffe „leichte Sedation“ und „tiefe Sedation“ mit Vorbehalt zu betrachten sind.
