Zusammenfassung
Der Einfluß von Isofluran auf die Kardiodynamik während akuter Volumenexpansion und auf die regionale Funktion des nichtischämischen und chronisch-ischämischen Myokards wurde an chronisch instrumentierten gemischtrassigen Hunden vergleichend zum Wachzustand ohne Sedierung untersucht. Anders als am isolierten Herz-Lungen-Modell war keine linksventrikuläre Dilatation, sondern eine Preloadabnahme zu beobachten. Während bei niedrigen Konzentrationen des Anästhetikums(l%) der Systemdruck durch akute Volumenexpansion noch gesteigert werden konnte, war unter 2% Isofluran, trotz größerer Preloadreserve als bei den wachen Tieren, keine Blutdrucksteigerung durch Flüssigkeitssubstitution zu erzielen. Im Rahmen der Hemmung kardiovaskulärer Reflexe in Inhalationsanästhesie zeigt sich bei dieser Konzentration auch ein fast völliges Sistieren der Regulation der Kardiodynamik über den Bainbridge-Reflex. Bei eindeutiger Abnahme der systolischen Herzleistung und der nichtischämischen Segmentarbeit kam es unter Isofluran zu einer milden Koronardilatation. Diese reichte jedoch unter den gegebenen Versuchsbedingungen nicht aus, im Vergleich zum normalen Herzmuskel, eine verstärkte oder permanente Dysfunktion chronischischämischen Myokardgewebes zu induzieren.
