Zusammenfassung
Der „Nagel“ besteht aus der Matrix, der von ihr gebildeten Nagelplatte, dem Nagelbett, den lateralen und proximalen Nagelwällen, dem Hyp- und Eponychium sowie der Kutikula. Die histologische Untersuchung des Nagelorgans wird nur von relativ wenigen Labors routinemäßig durchgeführt. Sie setzt ebenso wie die richtige Nagelbiopsie Grundkenntnisse der Anatomie, Physiologie, Wachstumskinetik und Pathologie voraus. Der Nagel ist schwer zu schneiden, und die Differenz in der Konsistenz zwischen Nagelplatte und Weichgeweben führt sehr häufig zu Rissen und Falten im histologischen Schnitt. Wenn nur die Nagelplatte untersucht werden soll, ist eine Formalinfixierung nicht erforderlich. Es empfiehlt sich, stets mit Hämatoxylin-Eosin und PAS zu färben. Die Fontana-Färbung ist bei Pigmentveränderungen oft unerlässlich, Giemsa und Trichrom-Masson geben Aufschluss über sonst nicht erkennbare Veränderungen des Nagelkeratins. Im polarisierten Licht ist die Anordnung der Keratinfilamente im Nagel erkennbar. Von vielen Nagelkrankheiten gibt es nur einzelne oder noch keine histologischen Untersuchungen.
