Pannikulitis

Zusammenfassung

Als Pannikulitis bezeichnet man Entzündungsprozesse im Panniculus adiposus des Unterhautfettgewebes. Klinisch imponieren Pannikulitiden als tief liegende Knoten oder plattenartige Indurationen. Prädilektionsstellen sind untere vor oberen Extremitäten, Unter- vor Oberschenkel-, Beckengürtel — und unterer Stammbereich. Rückschlüsse auf den Schwerpunkt des entzündlichen Geschehens in der Subkutis sind durch das klinische Verhalten der Knoten zu ziehen. Liegt die Entzündung hauptsächlich in den Bindegewebesepten (septale Pannikulitis) erfolgt entweder Restitutio ad integrum oder bei fibrosierender oder sklerosierender Defektheilung Verankerung der Haut in der Tiefe. Ist vorwiegend das subkutane Fettgewebe erfasst (lobuläre Pannikulitis) können fistulierender Durchbruch zur Oberfläche und Defektheilung in Form von eingezogenen Narben erfolgen. Sieht man von den direkt traumatischen Alterationen ab, reflektieren im Fettgewebe lokalisierte entzündliche Prozesse (Pannikulitis im engeren Sinn) oft schwerwiegende (systemische) Grundkrankheiten.
