Erworbene Vitien im Bereich der Aortenklappe

Zusammenfassung

Die operative Behandlung von Aortenklappenfehlern ist wesentlich später in Angriff genommen worden als die der Mitralleiden, wofür in erster Linie anatomische Gründe maßgebend gewesen sein dürften. Der erste entscheidende Schritt erfolgte 1951 durch Hufnagel [33], der zur Behandlung einer Aortenklappeninsuffizienz ein Kugelventil in die deszendierende Aorta implantierte. Herzarbeit, Ko-ronarzirkulation und Hirndurchblutung wurden mit dieser Technik jedoch nicht normalisiert. Auch Eingriffen an der Aortenklappe mittels geschlossener Verfahren, wie z.B. die Beseitigung von Stenosen durch instrumenteile transventrikuläre Valvulotomie [4] oder die Behebung von Insuffizienzen durch externe Raffung des Aortenfundamentes [57], war kein dauerhafter Erfolg beschieden. Erst die Möglichkeit zur Korrektur eines Klappenvitiums unter direkter Sicht mit Hilfe der EKZ machte die Entwicklung spezieller Operationstechniken für Aortenklappenfehler sinnvoll.
