Zusammenfassung

Die Chirurgie der Atrioventrikularklappen hat in den letzten 50 Jahren eine wechselvolle Geschichte durchlaufen, gekennzeichnet durch plötzlichen glänzenden Erfolg, aber auch durch lange Perioden der Stagnation und Entmutigung (Swazey u. Fox, 1970). Waren es in einem Fall hartnäckig verteidigte Fehlinterpretationen der Klappenpathophysiologie, die dem Fortschritt entgegenstanden, fehlte es im anderen an den nötigen technischen Hilfsmitteln, um richtige Vorstellungen in die Praxis umzusetzen. So herrschte in der ersten Phase theoretischer Überlegungen (Brunton, 1902) und erster operativer Versuche (Cutler, Levine u. Beck, 1923) die Fehlkonzeption vor, das Krankheitsbild der Mitralstenose ließe sich durch Schaffung einer Insuffizienz bessern. Andererseits schlug sich der richtige Vorschlag von Samways (1898) einer förmlichen Kommissurendurchtrennung nur in einem einzigen operativen Erfolg (Souttar, 1925) nieder, und es dauerte 25 Jahre, bis sich dieses Verfahren in der breiten Praxis durchsetzen konnte (Harken et al., 1948; Bailey, 1949; Brock, 1950).
