Zusammenfassung
Die Thrombasthenie Glanzmann ist eine angeborene Thrombozytenfunktionsstörung, die durch einen quantitativen und/oder qualitativen Defekt des Fibrinogenrezeptors Glykoprotein IIb-IIIa charakterisiert ist. Thrombozyten von betroffenen Patienten zeigen eine Störung der Thrombo-zytenadhäsion und der Thrombozytenaggregation bedingt durch eine Störung der Liganden-Rezeptor-Interaktion zwischen Fibrinogen/Fibrin und dem Glykoprotein IIb-IIIa. Bei normaler Thrombozytenanzahl und Thrombozytengröße wird die Diagnose durch eine quantitative und qualitative Analyse der thrombozytären Glyko-protein-IIb-IIIa-Komplexe gestellt. Die Blutungsneigung betroffener Patienten zeigt selbst bei identischem Mutationstyp eine erhebliche interindividuelle Variabilität. Leitsymptome sind Epistaxis, petechiale Blutungen und Menorrhagien. Mittel der ersten Wahl in der Behandlung von Blutungskomplikationen stellen lokale Maßnahmen einschließlich der lokalen Gabe von Antifi-brinolytika dar. Die Transfusion von Thrombozy-tenkonzentraten sollte wegen der erheblichen Gefahr einer Immunisierung einer strengen Indikationsstellung unterworfen werden. In jedem Fall sollten HLA-kompatible Thrombozytenkonzentrate eingesetzt werden.
