Zusammenfassung
Hauptursachen einer exokrinen Pankreasinsuffizienz, deren Nachweis an direkte oder indirekte Pankreasfunktionsprüfungen gebunden ist, sind in erster Linie die chronische Pankreatitis und das Pankreaskarzinom. Mögliche Ursachen einer exokrinen Pankreasinsuffizienz sind:

allgemeine Reduktion der Enzymsekretion:

chronische Pankreatitis,
-
Pankreaskarzinom (meist Pankreaskopfkarzinom),
-
akute Pankreatitis (meist passagere Insuffizienz),
-
Zustand nach Pankreasresektion,
-
Zustand nach Pankreastrauma (oft passagere Insuffizienz),
-
primär sklerosierende Cholangitis,
-
Hämochromatose,
-
Kwashiorkor, Proteinmangelernährung,
-
angeborene oder erbliche Störungen:
-
Mukoviszidose,
-
Shwachman-Diamond-Syndrom (exokrine Pankreasinsuffizienz mit Zwergwuchs, Dysostosen und chronischer Neutropenie).

Fehlende Enzymaktivierung im Dünndarm:

Enterokinasemangel.
