Zusammenfassung
Der Sekretin-Pankreozymin-Test (SPT) gilt als Goldstandard in der Pankreasfunktionsdiagnostik. In einer umfangreichen Studie an 403 Patienten fand Otte bei 8% falsch-pathologische und bei 6% falsch-normale Testergebnisse. Diese Prozentsätze werden sich kaum verbessern lassen, da die normale exokrine Pankreassekretion eine erhebliche Streubreite hat. Auch ist unklar, woran man falsch-normale und falsch-pathologische Testergebnisse messen muss. Beim Vergleich mit der endoskopischen retrograden Cholangiopankreatikographie (ERCP) wurde festgestellt, dass in einem Prozentsatz von 0–20% keine Übereinstimmung mit dem SPT besteht. Daher ist es fraglich, ob die ERCP in diesen Fällen wirklich den Zustand der Drüse am besten reflektiert oder ob Pankreasgangveränderungen nur Ausdruck von vernarbenden und abheilenden Entzündungsprozessen sind.
