Behandlungsprinzipien des Pneumothorax in Japan

Summary

A “pneumothorax study squad” was used to investigate a large number of patients in Japan, and the therapy could be standardized. The first episode is regularly treated by drainage; recurrence is then treated by thoracoscopical electrocautery. In the case of failure or massive bullae or adhesions, thoracotomy and resection are performed. The success rate of suction is 50 %, of thoracoscopy 83 %, and of thoracotomy 99 %. Therefore, drainage is justified for the first episode, but recurrence should be treated by minimally invasive surgery.

Zusammenfassung

In Japan konnte durch eine „Pneumothorax study squad“ anhand großer Fallzahlen ein Therapieschema standardisiert werden. Das Erstereignis wird mittels Thoraxdrainage behandelt. Bereits beim 1. Rezidiv erfolgt die thorakoskopi.sche Therapie. Bei weiterem Rezidiv, großen Blasen und/oder Verwachsungen wird. die Thorakotomie und Resektion durchgeführt. Der Erfolg beträgt für die Drainage 50 %, für die Thorakoskopie 83 % und für die Thorakotomie 99 % Rezidivfreiheit. Beim Pneumothoraxrezidiv wird deshalb ein minimal invasives Vorgehen mittels Thorakoskopie empfohlen.
