Summary
In the years 1978 to 1990, a total of 1233 resections of gastric carcinomas were performed at the Surgical Clinic of the University at Erlangen, Germany. While postoperative mortality after curative subtotal gastric resections could be derived from a deterioration of the patient’s general health prior to surgery (6 of 14) and from anastomotic leakages (p = 0.009), postoperative mortality after curative total gastrectomies was influenced by preoperative pulmonary dysfunctioning (p = 0.058), the preoperative presence of bleeding or stenosis (p = 0.017), the patient’s age (p = 0.032), the performance of a pancreatic left resection (p = 0.019), and anastomotic leakages (p <0.001). Postoperative mortality after curative subtotal gastric resections remained nearly constant (between 4 % and 6 %) over 13 years, while mortality after curative total gastrectomies was reduced from 15 % to 6 % during the same time.
Zusammenfassung
In den Jahren 1978 bis 1990 wurden an der Chirurgischen Universitätsklinik Erlangen 1233 resezierende Eingriffe bei Magenkarzinom durchgeführt. Während die postoperative Letalität nach kurativen aboralen Resektionen auf einen präoperativ reduzierten Allgemeinzustand (6 von 14) und auf Anastomoseninsuffizienzen (p = 0,009) zurückzuführen war, wurde die postoperative Letalität nach kurativen Gastrektomien durch die präoperative Lungenfunktion (p = 0,058), das präoperative Vorliegen einer Blutung oder Stenosesymptomatik (p = 0,017), das Alter zum Zeitpunkt der Operation (p = 0,032), die Durchführung einer Pankreaslinksresektion (p = 0,019) sowie durch Anastomoseninsuffizienzen (p < 0,001) beeinflußt. Die postoperative Letalität nach kurativen aboralen Resektionen blieb mit einem Prozentsatz zwischen 4 % und 6 % über 13 Jahre nahezu konstant. Die Letalität nach kurativen Gastrektomien konnte im gleichen Zeitraum von 15 % auf 6 % gesenkt werden.
