Diagnostik der Rektum- und Analatresie

Summary

Anorectal malformation in the newborn is still a challenge for the pediatric surgeon. First of all an exact diagnosis has to be made. The disadvantages of the invertogram proposed by Wangensteen and Rice are well known. In our patients, we perform an ultrasound scan using a 5-MHz linear probe, with the child lying on its back. A midline longitudinal scan through the symphysis shows the rectal pouch directly, thus permitting measurement of its distance from the anal verge. Air-filled fistulae and anomalies of the genitourinary traction can be detected during the same examination.

Das Neugeborene mit einer anorektalen Fehlbildung bedarf dringlich einer aussagekräftigen, möglichst nicht invasiven Diagnostik. Von wesentlicher Bedeutung sind die Höhenlokalisation des Rektumblindsackes, die Darstellung bestehender Fisteln sowie das Erkennen von Begleitfehlbildungen. Die Sonographie ist in idealer Weise geeignet, diese Fragen zu klären. Untersuchungstechnik^. 5 MHz Linearschallkopf, Rückenlage, medianer Sagittalschnitt durch die Symphyse, Lokalisation des Anus durch Zeigefinger eines 2. Untersuchers. Durch diese Technik ist die Distanz Rektum/Haut direkt meßbar.
