Operative Behandlung beim Magenfrühcarcinom

Summary

Data from our own clinical experience as well as overview articles from Japan show an 80%–100% 5-year survival rate for early gastric cancer. An indispensable prerequisite is a resection depending on the type, size and infiltration. For the sulbmucosal type, which shows lymphnode metastasis in 10%–25%, the appropriate surgical therapy is resection of the stomach or total gastrectomy with systemic lymphadenectomy of compartments I and II. Distinct types of mucosal carcinoma (small size) may be suitable for local therapy, particularly in high-risk patients.

Zusammenfassung

Sowohl eigene klinische Daten als auch Übersichtsarbeiten aus Japan zeigen für das Magenfriihcarcinom 5-Jahres-Überlebensraten zwischen 80–100%. Voraussetzung ist jedoch eine stadiumgerechte Therapie, abhängig von Typ, Größe und Infiltration. Für Frühcarcinome vom Submucosatyp bedeutet dies wegen positiver LK in 10–25% eine Magenresektion oder Gastrektomie mit Lymphknotendissektion der Kompartimente I und II (R 2-Resektion). Bestimmte Typen des kleinflächigen Mucosacarcinoms sind jedoch, abhängig vom klinischen Zustand des Patienten, evtl. auch einer Lokaltherapie zugänglich.
