Therapie der Ösophagusperforation sive Ruptur — operativ oder konservativ?

Summary

During a 7-year period, 23 patients were treated for perforations of the oesophagus (mean age: 54 ± 15 years): 8 patients underwent operative therapy and 17 patients were treated conservatively. Two patients had combined conservative and operative therapy. Conservative treatment consisted of transoesophageal suction (n =11), drainage of the thorax (n = 2), and only antibiotic therapy (n = 4). The transoesophageal catheter was inserted under X-ray guidance (n = 6), endoscopic guidance (n = 6), and without any control (n = 1). All catheters were removed after 16 days. There was only one death (9%) within the conservative treatment group compared to four deaths (50%) with operative therapy.

Zusammenfassung

In einen 7-Jahres-Zeitraum werden 23 Pat. (Alter: 54±15 Jahre) wegen einer Ösophagusperforation behandelt. 8 Pat. wurden operiert und 17 Pat. konservativ behandelt (2 Pat. wurden sowohl konservativ als auch operativ behandelt). 11 Pat. wurden mit einer transösophagealen Dauerabsaugung therapiert, 2 mit einer Thoraxdrainage und 4 ausschließlich antibiotisch. Die Katheter wurden unter radiologischer (n = 6), endoskopischer Kontrolle (n = 6) oder blind (n =1) eingelegt (bei 2 Pat. wurde wiederholt eine Sonde eingelegt). Alle Katheter wurden durchschnittlich nach 16 Tagen entfernt. 1 Pat., der konservativ behandelt wurde, verstarb (9%), im Vergleich zu 4 operierten Pat. (50%).
