Summary
Objectives of the study: using MRT to determine type, quality, and possible correlation to joint stability of anterior cruciate ligament (ACL) repairs or conservatively treated cases. Twenty-eight MRT were performed 3–96 months postoperatively. Results: Inter-rater correlation was 96%; an isolated suture repair restores only 11% of ACL to normal; no significant linear correlation was found between MRT and postoperative follow-up/stability tests; sensitivity 90.9%–100%; specificity 24%–43%; a type I ACL corresponds to good biomechanical function of the reconstruction; only satisfactory or poor short-term results following either conservative treatment (30% instability) or suture repair (39% instability). Clinical consequences: Isolated suture repair does not result in the reconstruction of a normal ACL; this technique should be abandoned. MRT is the only technique that allows a qualitative assessment of a reconstruction and secondary lesions at the same time.
Zusammenfassung
An 28 MRT des vorderen Kreuzbandes (10 konservativ therapierte, 18 mit isolierter Naht versorgt, Beobachtungszeitraum 3–96 Monate p.op.) wurde untersucht, wie sich diese Kreuzbänder in der MRT darstellen und ob die MRT mit klinischen und instrumentierten Stabilitätsparametern (KT 1000) korreliert. Ergebnisse: die isolierte Naht kann nur in Ausnahmefällen (11%!) ein normales VKB wiederherstellen; keine signifikante Korrelation zwischen MRT und p.op. Intervall/Stabilitätstests; Sensitivität 90.9%–100%; Spezifität 24%–43%; Typ I Band im MRT-Grading ist ein biomechanisch funktionsfähiges Band; befriedigende bis schlechte objektive Ergebnisse nach konservativer Therapie von Teilrupturen und nach VKB-Naht (Kurzzeit-Ergebnisse kons. 30%, Naht 39% instabil. Die MRT ermöglicht als einziges Untersuchungsverfahren eine Aussage zum Einheilungsverhalten eines autogenen Transplantates. Die isolierte VKB-Naht muß verlassen werden.
