Choledochuscystenresektion: Rekonstruktion durch cholangioduodenale Jejunuminterposition

Summary

The video begins with the ERC of an adult with a choledochal cyst Todani 4a. The operation starts with the preparation of the hepatic artery and portal vein. Cholecystectomy is followed by transsection of the cyst close to the duodenum. The cyst is dissected free from the hepatic artery and portal vein und finally resected centrally in the hilum of the liver. The reconstruction is performed by interposition of a 15-cm jejunal segment which is anastomosed side-to-end with the central bile ducts and end-toside with the descending part of the duodenum. Finally the video shows the control ERC 6 months postoperatively.

Zusammenfassung

Das Video zeigt einleitend die ERC bei einem Erwachsenen mit einer Choledochuscyste Todani 4a. Die Operation beginnt mit der Darstellung der A. hepatica und Pfortader und ihrer hilusnahen Aufzweigungen. Nach Cholecystektomie wird die Choledochuscyste am Duodenum durchtrennt, nach zentral von den Gefäßen abpräpariert und im Leberhilus abgesetzt. Die Rekonstruktion erfolgt mittels Interposition eines 15 cm langen Jejunumsegmentes, welches seit-zu-end mit den zentralen Gallengängen und end-zu-seit mit der Pars descendens des Duodenums anastomosiert wird. Abschließend zeigt das Video die Kontroll-ERC 6 Monate postoperativ, die unauffällige Anastomosenverhältnisse bestätigt.
