Zusammenfassung
Bei Patienten mit erworbenen oder kongenitalen operierten und nichtoperierten Vitien im Erwachsenenalter wird die individuelle Lebensqualität entscheidend durch die körperliche Leistungsfähigkeit beeinflußt. Da im Einzelfall eine erhebliche Diskrepanz zwischen subjektiver Leistungseinschätzung und objektiven klinischen sowie hämodynamischen Parametern beobachtet werden kann, ist eine Objektivierung der individuellen Leistungsfähigkeit mittels Belastungsuntersuchungen erforderlich [12, 14, 18]. Darüber hinaus sind Belastungstests wichtig bei der Verlaufs- und Therapiekontrolle, der differentialdiagnostischen Abklärung belastungsinduzierter Symptome, bei der Indikationsstellung zur weiteren invasiven Diagnostik oder zur Operation sowie bei der Wahl des optimalen Operationszeitpunktes. Besonders bei Adoleszenten und jungen Erwachsenen können Belastungsuntersuchungen eine Entscheidungshilfe bei der Berufswahl und der Wahl geeigneter sportlicher Freizeitaktivitäten bieten. Außerdem sind objektive Belastungsergebnisse gutachterlich, z.B. bei der Beurteilung von Berufs- bzw. Erwerbsunfähigkeit von Interesse.
