Zusammenfassung

Die Eigenschaft der Magenschleimhaut, den zerstörenden Wirkungen der Magensäure und des Pepsins zu widerstehen, ist die interessanteste und gleichzeitig auch wichtigste Eigenschaft der Schleimhautoberfläche. Obgleich das Wesen dieses Mechanismus nicht völlig aufgeklärt ist, besteht Übereinstimmung, daß das Phänomen einen multifaktoriellen Ursprung hat. Unter denjenigen Komponenten, denen diese Funktion überwiegend zugeschrieben wird, sind die visköse und schleimige Schicht des Mukus, die mit zäher Festigkeit an der Epitheloberfläche adhärent ist, die zellulären Membranen des Magenepithels und die Durchblutung der Mukosa [1–4]. Der Oberflächenschleim steht in einem dynamischen Gleichgewicht mit dem präformierten intrazellulären Schleim, der sich in den Sekretgranula des Oberflächenepithels und der Kryptzellen befindet, und gemeinsam bilden diese beiden Materialien die sog. Schleimbarriere des Magens [5, 6]. Während der Schleimschutzmantel die erste Abwehrlinie darstellt, repräsentieren die Zellmembranen des Magenepithels gemeinsam mit ihren “tight junctions”, ihren AbschluBleisten, die zweite Abwehrlinie.
