Zusammenfassung
In den westlichen Ländern haben die meisten Studien gezeigt, daß die Überlebensrate durch radikale Lymphadenektomie bei vertretbarer postoperativer Morbidität nicht verlängert wird [1, 2, 3]. Die Fünfjahresüberlebensrate beim Magenkarzinom beträgt seit den 50iger Jahren in Japan schätzungsweise 20%. Gemäß den Daten des nationalen Registers für Magenkarzinome in Japan besserte die kurative Resektion des Magenkarzinoms die Überlebensrate bis auf 75% im Jahre 1981 [4]. Diese bemerkenswerte Verbesserung hängt zum größten Teil mit den Bemühungen zur Früherkennung des Magenkarzinoms durch Suchtests zusammen und der Etablierung der adjuvanten Chemotherapie. In jüngeren Studien aus Japan betrug die Inzidenz des Magenfrühkarzinoms ungefähr 40% von allen Magenmalignomen [5, 6] und hat damit sicher die Überlebensraten für Patienten mit Magenkarzinom insgesamt verbessert. Mit anderen Worten: ein akkurates Bestimmen des Tumorstadiums ist für die Betrachtung der Überlebensrate notwendig (Tabelle 1).
