Zusammenfassung

Eine mit 150 IU/kgKG heparinisierte Patientin hat präoperativ eine Thrombozytenzahl (TZZ) von 3 · 105 und eine ACT („activated coagulation time“) von 200 s (ACT vor Heparingabe 100 s).

Intraoperativ sinkt die Thrombozytenzahl (TZZ) auf 3 · 104 ab. Angenommen, daß die restlichen Thrombozyten (TZ) voll funktionsfähig sind und daß keine Koagulopathie vorliegt,

  1. 1.

    ist die intraoperativ gemessene Blutungszeit (BZ) doppelt so lang wie die präoperativ gemessene BZ

  2. 2.

    muß man etwa 7 Thrombozytenkonzentrate verabreichen (keine weiteren Verluste), um die Thrombozytenzahl (TZZ) auf 105 anzuheben

  3. 3.

    normalisiert die Anhebung der Thrombozytenzahl (TZZ) auf 105 sofort die Blutungszeit (BZ)

  4. 4.

    muß man zusätzlich 200 IU/kgKG Heparin verabreichen, um die ACT auf 400 s anzuheben

  5. 5.

    muß man 50 IU/kgKG Protamin verabreichen, um die ACT auf 150 s zu verkürzen
