Zusammenfassung

Bei der TTT wird ein Diodenlaser mit einer Wellenlänge von 810 nm verwendet, der durch die Pupille auf das Zielgewebe gerichtet wird. Diese Wellenlänge penetriert gut durch Gewebe, nur ca. 5% der applizierten Energie wird von klaren Medien absorbiert. Die Energie wird im Wesentlichen vom Melanin retinaler Pigmentepithelzellen und Melanozyten in der Aderhaut aufgenommen und in Wärme umgewandelt (Berger 1997; Mainster u. Reichel 2000). Zur Therapie bei Aderhautmelanomen werden Herdgrößen von mehreren mm gewählt, und die Applikationsdauer beträgt Minuten. Die Behandlungsparameter bei der TTT unterscheiden sich damit wesentlich von denen bei der konventionellen thermischen Lasertherapie. Das Zielgewebe wird bei der TTT allmählich auf über 45 °C erwärmt, wobei die Temperatur unter der Koagulationsschwelle bleibt. Das Laserlicht wird über einen Spaltlampenadapter auf den Augenhintergrund projiziert, sodass das bestrahlte Gewebe während der Behandlung beobachtet werden kann (◘ Abb. 11.1).
