Hämodynamische Aspekte zu Alfentanil

Zusammenfassung

An 54 Patienten mit koronarer Herzerkrankung, die sich einer aorto-koronaren Bypass-Operation unterziehen mußten, wurde die hämodynamische und atemdepressive Wirkung von 20 ug/kg KG bzw.40 ug/kg KG Alfentanil mit der einer Kontrollgruppe verglichen. — Die Untersuchungen zeigten neben der Reduktion der Pulsfrequenz einen Abfall des arteriellen Druckes, des links-ventrikulären Druckes und des arteriellen Perfusionsdruckes während der extrakorporalen Zirkulation. Bei gleichbleibendem Herzzeitvolumen ist die Ursache hierfür in einer peripheren Vasodilatation zu suchen. Entsprechende Veränderungen sind auch unter Fentanyl beschrieben. Auch die Atemdepression (PaO2:-34%, pCO2:+29%) ähnelt der des Fentanyl, nur setzt sie früher ein und ist von kürzerer Dauer.
