Zusammenfassung
Die Dialyse ist eine weitverbreitete Therapie für terminale Nierenerkrankungen. Viele Jahre der Versuchs- und Forschungsarbeit sind mit der Entwicklung verschiedener künstlicher Nieren und extrakorporaler Blutreinigungsverfahren belohnt worden. Während die Dialyse früher nur einer ausgewählten Minderheit zugute kam, kann sie nun einer breiten Patientengruppe angeboten werden. Diese Zunahme spiegelt nicht nur eine Verbesserung der Technik, sondern auch eine recht eindrucksvolle Steigerung der Überlebensrate nach einem Jahr wider. 1964 lag sie bei 50%, 1979 stieg sie, trotz eines gegenwärtig höheren Durchschnittsalters und des Auftretens von Systemerkrankungen bei 10% der Dialysepatienten auf 86% an. Mit verbesserter Überlebensstatistik und der wachsenden Patientenzahl entstanden aber auch neue Probleme. Die derzeit führende Todesursache bei Dialysepatienten ist kardiovaskulärer Genese, wobei zerebrovaskuläre Zwischenfälle und Myokardinfarkte am häufigsten sind.
