Zusammenfassung

Aus der Pathogenese einer bakteriellen Meningitis läßt sich oft der ursächliche Erreger vermuten: Durchwanderungsmeningitiden, meist bei älteren Patienten, nach vorausgegangenen Schädelfrakturen, Otitiden, Mastoiditiden oder Nebenhöhleninfektionen werden häufig von Diplococcus pneumoniae verursacht. Infektionen, gegebenenfalls mit Liquorfisteln, nach vorausgegangenen neurochirurgischen Operationen müssen an Hospitalismuskeime (Staphylokokken, Proteus, Pseudomonas) denken lassen. Bei Patienten mit Immundefekten, konsumierenden Erkrankungen, Abwehrschwäche (Alkoholiker!) oder unter immunsuppressiver Therapie kommen seltenere Bakterien in Frage (Listerien, Streptococcus faecalis, Escherichia coli, Mykobakterien). Junge Erwachsene ohne zusätzliche Erkrankung erleiden meist eine Meningokokkenmeningitis. Andere Ursachen für bakterielle Meningitiden sind Osteomyelitis, Endokarditis und Sepsis.
