Endokrine Tumoren des Gastrointestinaltraktes

Zusammenfassung

Der erste Bericht über einen endokrin aktiven Tumor im Bereich des Gastrointestinaltraktes bzw. Pankreas stammt von Wilder et al. [39]. Sie beobachteten Hypoglykämien bei einem Patienten mit einem endokrinen Pankreastumor und demonstrierten die blutzuckersenkende Eigenschaft des Tumorextraktes. Seit dieser Beobachtung wurde zwischen funktionell aktiven Inselzellgeschwülsten und den Pathologen schon lange bekannten inaktiven Inseladenomen unterschieden. Es konnten bis heute bereits sechs klinische Syndrome ursächlich auf das Vorliegen hormonproduzierender Tumoren des Gastrointestinaltraktes zurückgeführt werden. Die funktionelle Aktivität derartiger Pankreasgeschwülste kann nur durch Hormonnachweis im Serum beurteilt werden, weil das Auftreten klinischer Symptome vom Ausmaß und nicht der Tatsache einer Hormonsekretion abhängt. Diese entspricht keineswegs der Tumorgröße.
