Zusammenfassung
Es gibt keine befriedigende Chemotherapie solider Tumoren des Gastrointestinaltraktes, und entsprechend kann keine medikamentöse Behandlung als Routinemaßnahme für die Praxis empfohlen werden. Ausgenommen sind die malignen Lymphome, Metastasen von Tumoren anderer Primärlokalisation, einige besondere Tumorlokalisationen wie die carcinomatöse Peritonitis oder sekundäre Tumorbeschwerden wie etwa das Postproktektomiesyndrom, für welche besondere Regeln gelten. Die (zumeist Adeno-)Carcinome des Gastrointestinaltraktes hingegen sind gegenüber Cytostatica generell so resistent, daß heute noch die Faustregel gilt, sie erst dann versuchsweise chemotherapeutisch anzugehen, wenn tumorbedingte Beschwerden des Patienten einen Behandlungsversuch mit potentiell sehr toxischen Mitteln rechtfertigen. Dem beschwerdefreien Patienten wird man nur bei besonderer Indikation Cytostatica zumuten.
