Zusammenfassung
Die Konstanz der chemischen Zusammensetzung der extracellulären Flüssigkeit ist Voraussetzung für die ungestörte Funktion der Körperzellen. Zu ihrer Aufrechterhaltung sind die Nieren als Ausscheidungsorgan unentbehrlich: Sie eliminieren fortwährend aus dem Blutplasma nicht mehr verwertbare Endprodukte des Zellstoffwechsels wie Harnstoff, Harnsäure oder Creatinin und scheiden sie in den Harn aus. Bei diesen Stoffen handelt es sich um „harnpflichtige Substanzen“, deren Verbleiben im Organismus zur Selbstvergiftung führen würde. Dasselbe gilt auch für viele aufgenommene Fremdstoffe (z.B. Medikamente), die oft nicht abgebaut und nur durch die Nieren wieder aus dem Organismus entfernt werden können. Die Nieren scheiden aber auch eine Reihe von physiologischen Bedarfsstoffen aus, wie Kochsalz, anorganisches Phosphat oder Wasser. Die Ausscheidung dieser Stoffe steht unter hormonaler Kontrolle und richtet sich nach den physiologischen Bedürfnissen.
